Final narrative
An employee was retrieving a mold from a racking system when the employee tripped over the support to an upright on the vacuum tube. The employee fell while holding the mold. Upon hitting the ground, the mold smashed the employee's right-hand index finger, resulting in a fingertip amputation.
An employee arrived at work and was walking into the store through the parking lot. The employee tripped over a curb and fell to the ground, sustaining a femur fracture.
