An employee was flushing the intake water of the fossil plant when a pipe broke loose on the unit and struck the employee on the elbow, breaking the employee's arm.

An employee was polishing the center of a try bar on a lathe when they sustained an avulsion of the right forearm that required surgery.

Two employees were lifting and installing the lance tube into place using a combination of chain falls and manual manipulation. While moving the lance tube into place, the employee on one end lost their grip on the tube and it fell to the grating. Since the lance tube was also suspended from a chain fall, the other end went up and caught the injured employee's right hand between the lance tube and an I-beam. The injured employee suffered lacerations across the top of their little, ring and middle fingers, and fractures to their little and ring fingers. The employee was hospitalized.

An employee was utilizing a truck mounted crane to remove a trailer hitch pin. As the employee was assisting with loading the haul trailer, the tongue of the trailer fell on the employee's foot. The employee's right foot was crushed.

An employee was moving a load to a work table. The load shifted and pinched the employee's left little finger against the table. The employee underwent a medical amputation to the finger.

An employee was inspecting components and contacts of a transformer load tap changer in a substation. When the equipment rotated, it caught the employee's left middle finger, causing an amputation at the nail bed.

An employee was tossing a damaged toilet into a compactor. The toilet bounced off the compactor and lacerated the employee's right wrist, cutting an artery and tendon.

S & J Construction, LLC

An employee was operating a hydraulic jack to bend a frame when the jack came loose and struck the employee. They were hospitalized for a spleen injury.

RMVK Enterprise, INC.

A truck had been towed into the shop with a flat tire and a blown airbag. An employee checked the tire for damage and was airing the tire. The employee was installing the valve core when the bead slipped off the rim. Air escaped, and the aluminum wheel struck the employee's head and face. The employee became unresponsive and was bleeding.

Dustrol Inc.

On December 14, 2023, at 8:40 p.m., three employees were inspecting milling machines for startup. A few of the sleeves on the mill drum needed to be changed out. The injured employee was trying to input a sleeve on the mill drum using a hammer. The hammer hit the edge of the drum, which caused a metal piece to splinter off and strike the employee's cheek just below the left eye. The metal splinter embedded in the back of their eye socket, resulting in pain, swelling, and bleeding that required hospitalization.

An employee was pushing a line of carts into a vestibule when the door cable snapped and struck his right ankle. The employee suffered a fracture that required surgery and hospitalization.

Employees were preparing to replace a primary electrical cable. While two of the employees were working to remove the cable on one side of a switchgear, they came into contact with a wrench and the outside part of the switchgear. One employee suffered an electrical shock.

El Paso Electric Company

An employee was walking to her work truck when she tripped over a safety cone and fell to the asphalt, resulting in a fractured tibia.

An employee was installing personal protective grounds at a substation when their hand contacted the end of the ground that was not energized, but induced voltage was created by wind and shocked them. This resulted in a burn on their thumb.

At about 9:35 a.m. on March 18, 2024, an employee was conducting maintenance on a non-operational fan controller in a switchgear room. The employee pulled the controller and began installing a new one. The employee was electrocuted, suffering third-degree burns to the right hand and fingers as well as burns to the right forearm, and was hospitalized.

An employee was working outside, sustained a heat-related illness, and was hospitalized.

United States Postal Service

A mail carrier was delivering mail when aggressive dogs approached. The dog's owner tried to restrain the dogs but one dog broke loose and the mail carrier pepper sprayed that dog. The dog's owner then pushed the mail carrier and he fell on the pavement and injured his neck. The mail carrier was hospitalized.

Swift Roofing, Inc.

An employee was installing a fall protection system on a flat roof when they tripped and became entangled in the warning line. They fell approximately 30 feet to the ground below. The employee sustained fractures.

U.S. Postal Service

A mail carrier developed heat illness while delivering mail. He was hospitalized.

Maxim Crane Works, L.P

An employee was outside assembling a 500-ton crane. The employee was connecting the rigging to the boom. He leaned forward to attach the lifting plate on the side of the boom and lost balance while bending over. He fell to the ground approximately 10 feet below, landing on his elbow. The employee was hospitalized with a fractured elbow that required surgery.

Viterra USA Grain, LLC

An employee was using a winch to secure a barge to a dock. The cable of an adjoining winch snapped and struck his left side, causing blunt force trauma and internal injuries to his abdomen.
