An employee was servicing a shear packer machine and sustained a partial finger amputation.

An employee was removing paper from a paper bag machine. The gears in the bundler amputated the employee's right index finger at the knuckle.

An employee was unjamming a paper bagging machine and amputated the tip of his left middle finger.

An employee's right hand was pulled into machine rollers, amputating the right index fingertip (above the knuckle).

An employee was cleaning bag machine plates with a rag when the rag and his left hand were pulled into the machine, amputating his fingertip between a plate cylinder and impression roller. The machine was running at the time.

Employees were assembling and testing a butterfly valve unit and actuator combo. When the unit was function tested with air, the valve gate closed on an employee's left thumb resulting in a fracture and laceration.

An employee was doing a daily inspection of the sifter screen on a mixer when the mixer line activated. The armature for the screen struck the employee's right wrist. Their wrist was cut and their hand was fractured.

An employee was being trained on a machine when a roller came down on their hand, resulting in a fracture and laceration.

An employee was doing a dry run during the setup of an ice cream machine. The machine indexed and the filler head amputated the employee's left middle finger above the last knuckle.

An employee was on a 3-foot ladder guiding cable onto a spool during installation on an overhead door. As the control box was jogged, the employee's hand was caught between the cable and spool, resulting in a wrist and hand fracture.

An employee climbed on a pallet to retrieve a "load flag." He fell while dismounting the pallet and landed on his right side on the floor. The employee sustained a fractured hip.

Employees were changing out a rubber roller in a laminating machine. The injured employee was supporting one end of the rubber roller while the other end was supported by a forklift. The end held by the employee slipped and fell onto their other hand which was resting on the equipment frame. They sustained an open displaced fracture of the distal phalanx of the right middle finger as well as an amputation of the right middle fingertip.

An employee was unclogging a dust collector on a binder machine. The machine amputated the employee's left middle fingertip.

An employee was troubleshooting a varnish pump. He removed and replaced the pump. The pump was turned on and the pump impeller amputated his right middle fingertip.

An employee was packing boxes when a small package conveyor disconnected due to loose bolts and hit her thighs. She suffered temporary paralysis in both legs and a swollen muscle.

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.

An employee was installing fence posts when they stepped into a recently dug hole that was not visible due to rainfall. The employee's leg was fractured.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee was inspecting a breaker box and turning on/off a circuit breaker when he was electrocuted.

An employee was walking on the sidewalk. When they stepped off the curb, they fell to the ground, resulting in fractures to their tibia, fibula, and a metatarsal.
