An employee was operating a 55-gallon drum mixing machine when his hoodie got caught in the auger pulling him into the machine. He was able to free himself as the belt broke on the auger. The employee then fell to the ground and had a seizure. The employee sustained contusions and two broken ribs.

An employee was attempting to switch a medium-voltage primary cable and install a 200-amp fuse barrel. The employee contacted the bottom of the switch gear cradle for the fuse barrel, causing an arc blast. The electricity entered the employee s left hand and exited his big toes, resulting in electric shock and burns to the left hand, arm, shoulder, and both feet. The employee was hospitalized.

On December 6, 2023, an employee of Duke Energy was working on a single-phase 120-/240-volt parallel service re-tap when a secondary flash occurred in an underground service. The employee suffered a second-degree burn to the face and was hospitalized.

An employee was operating a drill and tap machine. He was placing a part with his right hand and removing a part coming in on the left side. He saw a part that was crooked and went to straighten it. The machine clamped down on his left index finger, degloving the top part of the finger and resulting in a partial amputation.

As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members.
