An employee was moving a tool box when its top drawer started to open. While the employee was stabilizing the tool box and drawers, their left middle and ring fingertips were amputated through the nailbeds. The fingertips were surgically reattached.

An employee was assessing an overhead stamping press door for alignment issues when the counterbalance moved and their left thumb was caught between the chain and the sprocket. The thumb was amputated. The door was locked out/tagged out at the time but gravitational pull was not controlled.

He was then walking around the material train to validate it, and was struck by a forward-moving forklift. The forklift tire rolled over the employee's right foot/toes, fracturing four toes.
