An employee was servicing a restaurant machine and amputated the right index finger.
An employee was using a ladder in order to access the top of a walk-in cooler. The walk-in cooler was outdoors and the conditions were below freezing and icy. The employee slipped and fell approximately 5 feet to the ground, resulting in two fractured bones in the lower half of the right leg.
An employee arrived at the shop to pick up parts. He used a lift jack to move crates, uncrated a crate, took the parts out and set them on the side.
