An employee was driving a utility task vehicle (UTV) when it struck a load of steel angle iron that was resting on forklift forks, resulting in facial fractures.
At about 9:15 a.m. on May 22, 2023, an employee was using a 5-pound hammer to remove the weld on a pipe support and knock the support lose. The hammer missed the support; the employee's left index finger struck the support and was lacerated and broken. The fingertip was medically amputated.
