Final narrative
An employee was operating a horizontal boring mill when he stepped backwards and tripped over the tracks of the mill. He fell onto the concrete floor and fractured his hip, requiring surgery.
An employee was lying on his right side, using a torch, when sparks from the torch ignited his clothing. He suffered burns to the right side of his torso, chest, and back. He was hospitalized.
An employee was taking a wheel out of the Kort nozzle on a boat. The employee was pulling out the wheel using a crane and a come-a-long when waves hit the bottom of the boat and caused the load to shift and land on him, breaking his left foot/ankle and pinching his left ring finger.
An employee and a coworker were welding on the inboard and outboard sides of a bulkhead. Welding slag became trapped between the sweatshirts of one employee and ignited. The employee suffered first, second, and potentially third degree burns to the right side of the back and right armpit.
An employee was cutting sections of a vertical pipe (shaft for a valve). When he pushed the last cut section out of the way, the pipe fell straight down and pinned his fingers between the pipe welds, breaking his right-hand middle and index fingers.

He leaned forward to attach the lifting plate on the side of the boom and lost balance while bending over. He fell to the ground approximately 10 feet below, landing on his elbow. The employee was hospitalized with a fractured elbow that required surgery.
