An employee was nailing wooden boards against the rafters in an attic with an air-actuated framing nail gun when the recoil caused the gun to kick back. The gun hit the rafter behind him and then struck and penetrated the top of his left hand. The gun was in contact actuation mode at the time of the incident.

An employee was working in an attic securing a wire conduit. Due to high temperatures, he left the attic, became ill, and experienced shaking and cramping. The employee was hospitalized and treated for heat exhaustion.

An employee was installing wood trim surrounding a skylight inside a home. While holding a piece of trim in it's desired location, his pneumatic nail gun discharged, sending a 3/4-inch pin nail into his left forearm.

An employee removed a safety valve on a trailer that unknowingly contained nitrogen gas. The gas released from the trailer and pushed the employee backward off a scaffold approximately 10 feet before they struck a garage door, resulting in a pelvic injury.

An employee was sitting at her workstation when a nail gun malfunctioned and shot a nail into her left leg. She suffered a puncture and blood vessel injury and was hospitalized.

On November 25, 2023, an employee was using a high-pressured water hose to perform sewer line cleaning operations. The employee's right little finger and palm were lacerated by pressurized water from the hose.

An employee was changing fuel filters on a front-end loader. When he descended the access steps of the engine bay, he misstepped and fell approximately 3 feet to the concrete surface below. The employee sustained a fractured left hip.

While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.
