An employee was changing fuel filters on a front-end loader. When he descended the access steps of the engine bay, he misstepped and fell approximately 3 feet to the concrete surface below. The employee sustained a fractured left hip.

Employees were preparing to replace a primary electrical cable. While two of the employees were working to remove the cable on one side of a switchgear, they came into contact with a wrench and the outside part of the switchgear. One employee suffered an electrical shock.

An employee was assisting in the movement of a reel of steel wire onto a truck using a forklift and a sling. The load became stuck on a stanchion and the employee pulled up on the load, causing the employee's hand to become caught/crushed between the reel and the boom of the forklift. The employee sustained two hand fractures.

An employee was helping to offload a 267-pound cement load-bearing handhold from the flatbed of a truck when his left hand was caught under the handhold, injuring two fingers. He was hospitalized and had surgery.

An employee was replacing seal rings on a valve strainer. After isolating the valve and bleeding off steam, the employee was removing a plate holding the seal ring when water released within the strainer and valve. The water contacted hot metal, which caused it to steam. The steam contacted and burned the employee's stomach and upper legs.

A&S Services Group

An employee fell while exiting a trailer. The employee landed on their left side on the pavement, suffering multiple broken ribs on the left side. The employee was hospitalized.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

Lockheed Martin Mission Systems and Training

Two employees were removing a service lift platform from the back of a radar system. While handling the service lift platform, employee 2 was standing on the back of the radar and lost their footing, causing the service lift platform to shift and fall. The injured employee then fell from the top of the platform electronic subsystem (PES) to the floor, approximately 56 inches. The injured employee fractured his left elbow and left hip.

An employee fell from a loading dock to the ground 2-3 feet below. She suffered a compound fracture of the left hip and was hospitalized, requiring surgery.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee was standing on a step stool, removing the nuts and bolts from the frame of a solar panel that was being replaced. The employee's cheek made contact with a connector with damaged insulation. The employee was shocked, briefly lost consciousness, and fell to the ground, suffering an injury to the left shoulder.

An employee was walking in the warehouse when their foot was struck by the forks of an unloaded forklift that was passing by. The employee's foot was fractured.

An employee was climbing down an extension ladder from a residential roof. The ladder collapsed under his foot, and he fell backward onto the grass. He suffered a sprained neck, a fractured scapula, and fractures to his T7 and T12 vertebrae.

An employee was working to straighten a solar panel by reaching into the unguarded rotating section of the conveyor when their right thumb got caught in the conveyor, resulting in amputation of their right thumb.
