A temporary employee was cleaning the mill grinder floor when he fell approximately 20 to 40 feet through a hole in the floor and struck the floor below, breaking his right arm, pelvis, and right cheekbone.

An employee was riding a belted man lift when his fall protection became caught in the man lift. The employee fell approximately 8 feet and broke his lower left leg. The employee was hospitalized.
An employee was replacing a bearing on the shaker table. The injured employee and three co-workers were trying to place the shaker table back on the central pin, when the injured employee lost their grip and the shaker table came down on their right hand.
