An employee was loosening a loading spout and fell approximately 12 feet. The employee sustained a fractured right elbow and left tibia and a torn left ACL. Fall protection was not worn at the time of the incident.

An employee was riding a belted man lift when his fall protection became caught in the man lift. The employee fell approximately 8 feet and broke his lower left leg. The employee was hospitalized.

AG PROCESSING, INC.

An employee was replacing a bearing on the shaker table. The injured employee and three co-workers were trying to place the shaker table back on the central pin, when the injured employee lost their grip and the shaker table came down on their right hand. The right index finger was broken in two places and the middle finger was broken and lacerated. The injuries required surgery.

An employee was loading a rail car when he collapsed and fell. His fall was stopped by his personal fall arrest system (PFAS), but as he was already unconscious he struck his head on a beam or the rail car after his hard hat fell off. The employee was hospitalized for treatment of a seizure, lacerations, and head injuries.

A temporary employee was cleaning the mill grinder floor when he fell approximately 20 to 40 feet through a hole in the floor and struck the floor below, breaking his right arm, pelvis, and right cheekbone.

The employee was on top of a rail car to level the grain that was deposited into the car when they fell 13-15 feet to the ground. The employee sustained a right leg fracture.

An employee was on an extension ladder installing a satellite dish on a roof when they fell approximately 12-15 feet to the ground. The employee sustained fractures.

An employee was setting a 5-foot pry bar into a precast concrete slab and stood on the end of the pry bar to move the slab into place. The pry bar slipped out causing the employee to fall backward 15.5 feet to the dirt ground. The employee was hospitalized with a broken leg.

An employee was on a step ladder to measure and mark a crane rail for modification. The employee fell approximately 12 to 15 feet to the ground, resulting in a fractured right hip.

An employee was on a trailer securing a load of logs when they fell approximately 9-12 feet to the ground. The employee sustained brain hemorrhaging and lacerations to the thigh and above the eye. The employee was hospitalized.

American Vegetable Soybean & Edamame, Inc.

An employee was trying to redistribute the load on his trailer by adjusting/sliding the tandem wheels on the rear of the trailer. He started to pull the air release valve to release the pins, lost his balance, and had to put his hand on the tandem frame rail. The rail system caught and amputated the tip of his right middle finger.

Owensboro Grain Company

An employee was getting the barge ready for a load of soy beans. The employee opened the top door of the grain barge and lost his balance, falling 10 to 14 ft. to the bottom of the barge and resulting in blunt force trauma and multiple broken bones.

PURIS Proteins, LLC.

On December 23, 2020, an employee was cleaning overhead piping using an extended brush on an 8-foot A-frame ladder. The ladder began to tip and the employee fell to the floor, suffering two broken bones in the ankle that required surgery.

Perdue AgriBusiness LLC

An employee was watching the flow of product in a meal load sampler. The automatic sampler in the machine struck the employee's left arm, causing a break that required hospitalization and surgery.

An employee was riding a belted man lift when his fall protection became caught in the man lift. The employee fell approximately 8 feet and broke his lower left leg. The employee was hospitalized.

An employee was readjusting a safety hook to remove hardware from gang forms when his feet slipped. He fell to the ground, resulting in a broken left arm and wrist.

An employee was on a step ladder in a store aisle, making room on shelving for merchandise. She fell from the step ladder to the floor, suffering two fractures in her left leg. She was hospitalized, requiring surgery.

Corrigan Brothers Inc

An employee was closing the side cargo door to a company van. The tip of his finger got caught between the door and the door jamb, resulting in a partial amputation.

Plumb Supply Company

An employee lost his footing while climbing an inclined ladder. He stumbled down one step and was able to stabilize himself by holding the railing and twisting his body, but he suffered an internal abdominal injury and was hospitalized.

JRI INDUSTRIES

An employee was manipulating dies on a brake press. The press caught and crushed the employee's right forearm, resulting in hospitalization.
