While the employee was holding the insulation hose, it caught on the guardrail of the catwalk/suspended walkway leading into the attic. The tugging motion caused the employee to lose their footing and step onto the sheetrock ceiling. The employee fell through the ceiling to the ground about 8 feet below and suffered a broken lower left leg. The employee was hospitalized.

An employee was going between a ladder and a wall to repair a wall in the utility room. A brad-nailing gun was hanging on a piece of wood on the side of the ladder. The employee contacted the brad nailer and the trigger was pushed into the wooden piece. The brad-nailing gun actuated and shot a 2-inch brad into the employee's abdomen.

An employee was on a scaffold caulking and painting a manufactured home. She fell off the scaffold an estimated 6 to 8 feet, landing on the concrete floor. The employee suffered upper body injuries and was hospitalized.

An employee was going up a ladder from the first level roof to the second level roof. The ladder slipped and the employee fell approximately 8 to 10 feet to the surface on the first level. The employee sustained fractures to three ribs and bruises on their arms and hands.

As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.
