When he stepped off the table with his left leg, his left knee gave out and he fell to the ground, landing on his right hip. The employee sustained a displaced right hip fracture near the joint and was hospitalized.

An employee was pressure washing a suspended part when the part started spinning. The employee manually stopped the part from spinning when his hand was caught in the pressurized water blast. The employee was hospitalized with a laceration.

An employee was dumping a "self-dumping chip bin" (on the forks of a forklift) into a 3- or 4-foot-tall dumpster. The bin tilted forward. Thinking it was going to slide off the forks, the employee reached to grab the bottom of the bin to stop it. The bin then rotated back. The tip of the employee's left middle finger, from the first knuckle past his fingernail, was caught between the bin and the top of the forklift pocket frame. The fingertip was amputated just above the first knuckle.
