While unloading a reel of wire with a pallet jack, the wheels of the pallet jack rolled off the truck's lift gate causing the wire and the pallet jack to flip over onto the ground and strike the employee's ankle and fracturing it.

An employee was troubleshooting an open circuit at the solar panel power generating area. When he disconnected the connectors from the power generating panel an arc flash occurred., causing third-degree burns to the tops and palms of each of his hands.

An employee was moving a steel-plated skate/equipment pallet across the floor. The employee's right middle finger was caught between the skate and the floor. The fingertip was amputated with no bone loss.

An employee was repairing the right rear tire of a tractor in a customer's yard. While removing the cribbing, the tractor slipped and came down on him. The tire was not yet inflated and the employee was pinned by the tractor. The employee sustained crush injuries to the hip, pelvis, and tailbone including several fractures to the pelvis.
