An employee was installing a combat vehicle turbine engine to a dynamometer stand in a dynamometer test engine cell. While lining up the turbine engine to the stand, the employee's left arm was fractured between the engine and the face of the stand.

An employee was rigging the front of a heavy armored vehicle to offload from a railcar. The employee's right middle and index finger were caught between the rigging hook and the vehicle's engine cover latch hook, causing the amputation of flesh from both fingers.

An employee was securely elevated in a bucket truck when they received an electrical shock from a high-voltage power line. The employee fell down in the bucket after he sustained the shock. The employee then stood up and his basket was lowered to the ground. The employee was hospitalized with burns to both arms and hands.

An employee had a heart attack during a 2-mile walking event and was hospitalized.

An employee was pulling on a stackable metal bookshelf while a second employee was pushing it. The first employee slipped and fell to the floor, fracturing the T12 vertebra and requiring hospitalization.

An employee was fixing ceiling tiles while working from a ladder when they fell to the floor and sustained a hip fracture.

An employee was on an A-frame ladder retrieving materials from a shelf. He slipped and fell and his leg got caught in the ladder rungs, breaking his left tibia.

An employee was unloading materials from a flatbed trailer when they fell from the trailer and landed on the ground. The employee sustained a fractured left leg.

An employee was unloading appliances at a customer location for delivery. The employee was moving a water heater to be placed on the liftgate when he stepped backward and fell to the ground. The employee sustained rib and back injuries.

An employee was coming down a portable ladder when they slipped and fell to the ground, resulting in a right forearm fracture.

On May 4, 2025, at approximately 11:00 AM, an employee was using a table saw to cut notches in a piece of wood when the equipment kicked back, contacting their left index finger. The employee sustained an amputation of the fingertip.

On November 7, 2024, at about 12:30 PM, an employee was moving a cannon on a gantry crane when the straps holding the cannon broke and it fell. The employee was hospitalized with a crushed pelvis, multiple fractures, and other severe injuries.

An employee in the welding training booth was working to back gouge a weld on a 5mm plate. While grinding, the gouger high-centered and walked over the plate striking and severely lacerating the employee's left thumb.

An employee was walking when his foot caught residual adhesive from floor tape and he tripped and fell to the concrete floor, resulting in a fractured right hip.

An employee was adjusting a switch on the loading cradle for a metal tracer lathe. The machine cycled, resulting in an amputation to the left index fingertip.

An employee was attaching a steel angle plate weighing (approximately 200 pounds) to the end of a structural member (5,000 pounds) and was supported by dunnage. The dunnage was a crane mat (11 inches tall) with a 4x6 board on top of it and it supported five identical members. The main piece was 30.75 feet long, 37.25 feet tall, and 1 food wide. As the employee slid the piece of metal on top of the member to begin attaching it, the entire member began to tip over. The employee went to stop the main piece from tipping over and it landed on his ankle, fracturing it.

An employee was preparing hot melted butter on a flat-top grill and placing it in small containers once it was melted. As she turned to set the container down, it slipped out of her hand and landed on the grill. Butter splashed up and hit her in the face. The employee suffered second-degree burns to her right eye and the right side of her head and face, as well as first-degree burns to her right forearm.

An employee pulled into the customer's driveway, parked, and exited the vehicle. He opened the sliding side door on the vehicle to retrieve a package. The customer began backing their vehicle out of their garage and it struck the employee in the back of his left leg. He was pinned him between the two vehicles, causing contusions and lacerations.

An employee reached into a hamper, picked up a package, and was hospitalized with a back injury that caused pain in his lower extremities.

On July 10, 2025, an employee tripped over a yellow pillar and fell to the floor. The employee was hospitalized with a fractured left femur.
