At about 10:00 a.m. on July 6, 2021, an employee was doing electrical powerline work when he developed heat stress symptoms. He was hospitalized.

An employee had been climbing a utility pole to install a dolly on the pole and fell approximately 50 feet to the ground below. The employee was hospitalized with a lacerated spleen and internal bleeding.

While pulling electrical line using a tugger and tensioner, an employee came in contact with a live electric current and sustained burns to the left hand.

An employee opened an energized PMH (large electrical box used for power transmission) to verify tags and labels and an arc blast occurred. The employee received first and second degree burns to his right arm, arm pit, shoulder, ear and neck.

An employee was spotting equipment being moved by a rough terrain forklift when the forklift ran over the employee's foot, breaking it.

An employee was preparing a tank car for loading while chipping away solidified product in the manway opening of the tank car. A pressure noise within the tank car startled the employee, causing him to stumble backward and fall approximately 10 feet over the guard railing at the chemical loading rack to the ground. The employee suffered fractured vertebrae, a fractured right wrist and ankle, and a dislocated jaw.

An employee was installing an electrical tray remover from a ladder. As he was descending, his missed a step and fell 6-7 feet to the floor. The employee suffered a head injury.

Two employees were installing spray foam insulation in the attic of a new residential construction. The injured employee fell 8 feet from an A-frame ladder to the floor. The employee suffered lacerations, abrasions, and a head injury.

An employee was on a raised platform attaching wires to electrical boxes. The employee fell approximately 8 feet to the floor below and struck an exit sign on the way down, resulting in an injury to his face.

An employee was in a forklift man basket fixing a garage door opener when they fell approximately 8 feet to the concrete ground below. The employee suffered head and back injuries as well as kidney bruising.

An employee was terminating cables in a junction box. A loose ground wire came into contact with the bushing, causing a flash that burned the right side of the employee's face and his right hand.

An employee was using a bucket truck hoist to raise secondary aerial wiring. The wire made contact with the primary wire, causing an arc flash. The employee suffered burns to both hands and was hospitalized.

An employee was working to move a telephone pole when the pole rolled, causing injury to their hip.

An employee was terminating conductors to buss bars inside the secondary compartment of a single-phase transformer. An arc flash occurred, causing burns to the soft tissue of the employee's face.

An employee was securing the claw of a grapple truck to the truck bed. His left little finger was caught between the tie down strap and the rub rail of the truck, resulting in partial amputation of the finger.

An employee was checking on a machine outside of the building and removed a cover to clean out a blocked area. His hand was caught by a rotary valve in a dust collection machine. The employee's fingertip was amputated.

An employee was working to clean a glue roller with a scraper tool. The tool and the employee's left thumb were pinched between two rollers, resulting in a partial amputation.

An employee was re-arranging boxes of pipe insulation on a pallet. One of the boxes fell from the pallet and knocked over an upright empty cylinder. The cylinder fell and crushed the tip of the employee's right toe. The employee's toe required surgical amputation.

An employee was securing the claw of a grapple truck to the truck bed. His left little finger was caught between the tie down strap and the rub rail of the truck, resulting in partial amputation of the finger.

An employee was cutting extruded aluminum framing material using a horizontal band saw. She went to grab a rag in the machine when the rag contacted the blade and pulled her hand in toward the blade, resulting in a partial amputation of the right index finger.
