Final narrative
An employee was stacking bags of beans in a railcar and was standing on them when the stack shifted and he fell to the railcar floor. He suffered a lacerated forehead and fractured right wrist, requiring overnight hospitalization.
The injured employee was cleaning in the mill room and was standing on top of a guardrail on a platform. A forklift operated by another employee struck the platform and the injured employee fell 6.5 feet from the guardrail he was standing on to the mill room floor below. The employee fractured both heels and the left ankle.
An employee was on an A-frame ladder retrieving materials from a shelf. He slipped and fell and his leg got caught in the ladder rungs, breaking his left tibia.
An employee was unloading materials from a flatbed trailer when they fell from the trailer and landed on the ground. The employee sustained a fractured left leg.
