Final narrative
An employee was getting ready to repair a broken PVC pipe that was dripping water on the floor. He placed blankets in the area to contain the dripping water and then began climbing a ladder to the pipe when he slipped and fell off the ladder, fracturing his hip.
On August 29, 2015, at approximately 5:00 p.m., an employee was unloading material using a built-in hydraulic lift gate attached to the rear of the delivery truck. The material fell on and broke the employee's pelvis.
An employee was on an A-frame ladder retrieving materials from a shelf. He slipped and fell and his leg got caught in the ladder rungs, breaking his left tibia.
