Final narrative
An employee went to close the roll-up door to the trailer. There was a strip of metal at the bottom of the door which was attached to the cable that acts as a counterweight to the door. As the door was pulled down, the metal strip became detached from the door and was pulled upward toward the top of the door by the cable. As this occurred, the employee was cut by the metal strip across both of his wrists and required surgery.

An employee was mounting a tire to a wheel using a tire changing press. As the machine's robotic arm was lowered into position, it caught his left ring fingertip against the wheel, resulting in amputation.
A crew was pulling a 4-inch plastic gas pipe off a reel and straightening it for installation. The injured employee stepped up on the trailer to cut the last band holding the pipe in the coil on the reel.
