An employee was walking in a corridor when she tripped and fell on the floor. The employee was hospitalized with fractures to her left shoulder and left ribs, and she sustained an injury to her left elbow.

An employee was taking wrenches from a shop and loading tools into a pickup truck. When he turned around from the truck, he tripped over skid-steer tracks that were loose on the ground and fell. The employee's arms were fractured and their right arm was also dislocated.
