An employee was walking on site when he tripped on a wooden survey stake that was lying flat on the ground. He fell into another employee and sustained a fracture to the tibia/fibula.

An employee was working on top of a rebar mat that was about 4 feet off the ground. As he walked across the mat, he stepped into a gap and his lower leg was caught in the rebar. His right leg twisted, and he suffered fractures to the tibia and fibula. He was hospitalized.

A Zachry Industrial employee was pushing a cart up a walkway. A forklift struck the employee as it finished turning around, its back right wheel hitting the employee's left foot and causing multiple fractures.

An employee was welding the bottom of a chute on the underside of a bin. Hot material came through a small gap between a gate inside the bin and the chute, contacting the employee below. The employee received second and third burns to the neck.

An employee was working to fit two 1-1/2-inch stainless steel spool pieces at a flanged connection to bolt up. To align the two spool pieces, the top section of pipe (10 feet in length) was rigged using a nylon strap choker, attached to a 1/2-ton chain. As the employee attempted to move the pipe, the load shifted in the nylon strap, causing the top section of the pipe to drop approximately 1-2 inches, which pinched the injured employee's left index finger in the flanged connection and resulted in an amputation of the finger's distal tip.

An employee was installing wood trim surrounding a skylight inside a home. While holding a piece of trim in it's desired location, his pneumatic nail gun discharged, sending a 3/4-inch pin nail into his left forearm.

An employee removed a safety valve on a trailer that unknowingly contained nitrogen gas. The gas released from the trailer and pushed the employee backward off a scaffold approximately 10 feet before they struck a garage door, resulting in a pelvic injury.

An employee was sitting at her workstation when a nail gun malfunctioned and shot a nail into her left leg. She suffered a puncture and blood vessel injury and was hospitalized.

On November 25, 2023, an employee was using a high-pressured water hose to perform sewer line cleaning operations. The employee's right little finger and palm were lacerated by pressurized water from the hose.

An employee was hydroblasting when they slipped and the pressurized water stream lacerated their left ankle.

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

An employee was climbing a ladder to inspect a vent in a restaurant dining area. The ladder slid out on the floor and the employee fell approximately 12 feet onto the ladder and the floor. He was hospitalized with a dislocated shoulder and a back injury.

An employee was operating a lift truck when it struck a stationary lift truck. His left thumb was pinched between the two lift trucks, resulting in a partial amputation.

An employee was disassembling a pressurized unit using a driver drill. When the unit was disassembled, it caused a kickback from the tube head, causing the drill to strike the employee's left forearm. They sustained a diaphyseal fracture to their left radius and ulna, which required surgery.

After repairing a piece of industrial equipment, an employee had brake-cleaning liquid on his gloves. A lighter ignited the liquid, and the employee's hands were severely burned.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.

An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.

An employee was operating an agricultural tractor during sugarcane harvesting. The employee sustained a lumbar sprain due to vibration or motion from the tractor.

An employee was moving a 3-ton condensing unit, strapped down on a dolly, out of a garage. The strap broke, causing the employee to fall backward onto the brick pavered driveway. The employee suffered injury to a spinal ligament in the neck.
