An employee was standing on a 6-foot platform ladder while attempting to get his welding wire to feed properly through his weld wire feeder. He fell off the ladder and landed on the hook of a torch cutting station stand, suffering a laceration that was approximately 5 inches long and required surgery.
An employee was moving a metal skid onto a skid flipper and the skid was not lined up with the center of the clamps. The employee went to move it into position with a rigging bar when the skid was pushed from the back. The employee's right index finger was caught between the skid and the clamp of the skid flipper and was partially amputated.
