On December 6, 2017, at approximately 9:15 a.m., an employee was climbing a 12-foot step ladder to attach a retractable lanyard to a structural member when the employee fell backward from the fourth rung. The employee's head struck the concrete floor, and the employee was hospitalized with a skull fracture, brain bleed, and abrasions to the lower left calf and ankle.

On January 23, 2025, an employee was performing maintenance work on an HVAC unit at a client site. As he was reaching into a tight space to access a mechanical water valve, his hand grazed the end of an unprotected live electrical wire. He suffered an electrical burn.
An employee was on an A-frame ladder retrieving materials from a shelf. He slipped and fell and his leg got caught in the ladder rungs, breaking his left tibia.
