Federal OSHA recorded a severe workplace injury at UL LLC, 333 PFfingsten Road, NORTHBROOK, ILLINOIS 60062 on — Fractures, affecting the chest, except internal location of diseases or disorders.

An employee was using a crane to lift a filtration tank filled with sand to the main level of a hydro lab when the tank fell. The employee was struck by the falling tank, an electrical conduit, and a metal pipe, suffering multiple broken ribs, swelling to the right leg, and a crushing injury.

An employee was cutting an aluminum bracket with a miter saw when the bracket pinned his finger up against the backstop, resulting in fingertip amputation.

Star Pipe Products LTD

An employee was using a crane. The hook slipped and a fitting fell onto a finger on the employee's left hand causing a fracture and laceration.

KP Building Products, Inc.

An employee was transporting an 800-pound die on a cart from the tool room. The employee was pulling the cart when it struck a metal plate on the floor. This caused the cart to tip forward and the die to slide off the cart. The die struck the employee's left foot and their second toe was amputated.

BJ'S WHOLESALE CLUB, INC.

An employee was using a forklift to load pallets of product on a trailer. He exited the forklift to adjust a pallet by hand. The pallet fell on him, resulting in an injury to the left leg.

International Steel and Counterweights LLC

An employee was lifting a 500-pound steel counterweight off a pallet using a magnetic hold jib crane. The counterweight detached from the crane and the employee sustained a left foot/toe fracture that required surgery.

Seneca Mechanical, LLC

An employee was using a pipe wrench to turn a 42-foot-long steel pipe on jack stands to weld the bottom portion. The pipe fell off the stands, striking the injured employee on his left shin. The employee sustained a left leg fracture at the shin area as well as a tibia fracture that required surgery.

MAV-TEC Testing Laboratories, LLC

An employee was operating a horizontal belt sanding machine that rotates a metal sanding belt. The employee switched hands while the belt transitioned from left to right. When the machine's speed was adjusted, the employee s left hand was pulled into the machine and crushed. The employee was hospitalized and required surgery.

Boeing (LMTF)

An X-ray machine had lost power and an employee went to replace the power supply. While replacing a cable on the power supply assembly, the employee's right hand contacted the grounding terminals coming out of the back of the power supply assembly and they received an electric shock. The employee was hospitalized.

Cape Cod Healthcare Lab Services

On March 19, 2025, an employee was retrieving a medical specimen from the back of a vehicle. When he turned to enter the building, he either tripped over something or lost his footing and fell due to an incline in the pavement. The employee was hospitalized with a head abrasion and injuries to his right shoulder and right hip.

Agilent Technologies - Frederick, Colorado

Employees were pulling a chemical sample from a device (sample port). The sample wouldn't come out because there were two samples in the port. The employees then turned off and deenergized the machine to 0 pounds per square inch. They unscrewed the top of the machine to retrieve the stuck samples when a small amount of pressure caused liquid to squirt out and onto one of the employee's face shields as the lid was removed. The employee was hospitalized due to chemical exposure.

An employee was walking around a boiler recovery tank when they stepped into an uncovered U-drain filled with hot water. The employee sustained second-degree burns to their right leg and the top of their foot.

Takeda Pharmaceuticals U.S.A., Inc.

An employee fell while moving a water canister, resulting in fractures to four fingers on her right hand.

An employee was changing a die in a press when the die slipped and crushed the employee's left index finger. The employee sustained an open facture of the tuft of the left distal phalanx and a partial amputation.

An employee was mixing sauce using a sauce barrel mixer when his left little finger was amputated.

AbbVie Inc.

An employee was driving a boom lift (in the lowered position) in an exterior dock area. The lift s left wheels rolled onto base plates that covered a 3-foot-deep sump pit. The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.

A temporary employee was carrying a nail gun through the warehouse when it hit his leg, activated, and fired a nail into the leg.
