An employee was helping a customer place a trailer hitch onto the hitch ball. While determining why the hitch would not go onto the ball, the employee placed his index finger up into the hitch. At the same time, the customer placed his vehicle in park and took his foot off of the brake. The vehicle moved and pinched the employee's index fingertip, resulting in amputation.

An employee was helping a crew erect scaffolding. As he applied pressure to a component, it gave way and causing the employee to fall from the scaffold to the ground. The employee sustained a dislocated elbow that required surgery.

On August 31, 2023, an employee placed a hydraulic controlled attachment on a skid-steer. The attachment fell off and contacted the employee's left leg, resulting in an injury.

Employees were loading pipes onto a flatbed truck when one of the pipes rolled off the truck and struck the injured employee, causing a fractured vertebra.

On December 21, 2022, an employee was loading and unloading a flatbed truck. The employee jumped from the ground to get onto the trailer but did not clear the trailer and his knee struck it. The employee sustained a laceration to his knee.

An employee was operating a sit-down type of material handling equipment (MHE). They were turning the MHE with the clamp attachment in the air when the MHE fell over onto its right side. The employee's right foot was caught under the battery cover of the MHE. The employee sustained a chemical burn from the battery and a fractured right ankle.

An employee was driving a snowmobile when it flipped over. The employee suffered a broken rib.

An employee was driving an unloaded forklift outside a building. During a left turn, the forklift drove off the paved area onto an unpaved surface, became unstable, and tipped over. The employee was pinned under the forklift and suffered two broken leg bones and a broken hip.

An excavator bucket was being removed. An employee was pulling a snap ring to remove a pin and lost his grip. The high-tension snap ring recoiled and his left middle fingertip was amputated by the snap ring.

While using the remote operator controls and walking alongside the lift, the wheel of the lift turned in the wrong direction, causing it to strike the employee and roll over his left ankle. The employee was hospitalized with two broken bones in his ankle.

An employee was working on an excavator's tracks. He was swinging a sledgehammer to finish pushing a metal tractor pin into the track links. A hammer-blow caused a piece of the pin to fly off, ricochet, and embed itself in his buttock. He was hospitalized, requiring surgery.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.
