An employee was using an air-actuated pump to pump oil from a mill to a 55-gallon drum when the pump malfunctioned. The employee went to fix the pump when it activated and the piston contacted their left index fingertip, resulting in an amputation.

On June 13, 2025, an employee was applying sprayed concrete to the top of coke ovens in a tunnel that is typically around 200 degrees. Later in the day, they began to show signs of heat-related illness, resulting in hospitalization.

Employees were raising a structural steel beam (L 27' x W 24" x 104") when it fell striking the injured employee. They were lifting the beam using a beam clamp and a crane to orient it to slide in place to be bolted up. The injured employee's lower left leg was amputated. The employee also sustained serious injuries to the right leg, a bruised hand and a possible fractured cheekbone.

An employee was setting trusses at a building when he slipped and fell 5 to 8 feet, dislocating his ankle. Fall protection was worn at the time of the incident.

An employee was cutting a piece of plywood on a table saw when his left glove and hand were caught and pulled into the saw blade, amputating his index and middle fingers and lacerating his ring finger.

An employee was climbing a step ladder while carrying a 3-foot piece of conduit. As he went to reposition his feet on the ladder by pivoting, he slipped and fell from the third rung of a 6-foot ladder. The employee sustained fractures to the left femur, right elbow, and right ring finger.

An employee was descending a 4-step maintenance stand when she missed the bottom step and fell to the hangar floor. The employee suffered a left hip fracture.

An employee stopped a belt and was going to check it for missing packages. She fell backward 3-4 feet from an elevated platform and sustained a broken right arm.

An employee had just finished a routine concrete pour and was ascending the ladder to clean the concrete mixer truck. The employee lost their grip and fell approximately 2-3 feet, contacting the truck's bumper. The employee suffered rib fractures and a punctured lung.

An employee was performing duties as an expeditor. After opening the dock door, the employee scanned the barcode on the door of the truck and placed one foot on the truck and one foot on the dock. The truck drove out of the stall, causing the employee to fall 4 feet off the dock onto the concrete. The employee sustained fractures to the right side of the pelvis, elbow, and a left ring fingertip as well as injuries to the right wrist and bruising to the back and stomach.

An employee was terminating cables in a junction box. A loose ground wire came into contact with the bushing, causing a flash that burned the right side of the employee's face and his right hand.

An employee was using a bucket truck hoist to raise secondary aerial wiring. The wire made contact with the primary wire, causing an arc flash. The employee suffered burns to both hands and was hospitalized.

An employee was working to move a telephone pole when the pole rolled, causing injury to their hip.

An employee was terminating conductors to buss bars inside the secondary compartment of a single-phase transformer. An arc flash occurred, causing burns to the soft tissue of the employee's face.

An employee was securing the claw of a grapple truck to the truck bed. His left little finger was caught between the tie down strap and the rub rail of the truck, resulting in partial amputation of the finger.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.

An employee was inspecting flares processes. The employee received burns to the front side of the body, face and arm from the flares.

An employee was removing plastic material from a production line when the machine cycled and amputated his left index, ring, and little fingers.

An employee was walking up a walkway to enter a building through the back entrance when she tripped over the lip of the cement ramp. The employee fell and sustained a fractured right hip.
