An employee was struck in the head by a fitting that fell when a contractor was working overhead, requiring hospitalization for a concussion.
An employee climbed on top of a roller conveyor to visually assess a suspected fractured pallet that was being conveyed to the finished products warehouse. The employee's safety toecap of his boot got caught in a 4-inch-wide gap separating sections of the conveyor rollers, causing him to twist his leg and fall. The employee suffered a fractured femur and finger.
An employee was exiting a jockey truck when his foot got stuck on a step, causing him to fall to the ground. The employee sustained a fractured right leg.
A mechanic was performing preventative maintenance on a robot. The employee noticed the right grease port next to the pallet pusher bar was blocked, so they notified the operator.
