Employees were disassembling a blowout preventer. During the process, one of the employees was attempting to investigate a blockage when the 500-pound BOP dropped onto his right arm between the wrist and elbow. He received a laceration and a fracture, requiring hospitalization.

An employee was using a sledgehammer at a drilling location. The employee's finger was crushed between the hammer handle and a pad eye. The employee was transported to the hospital and the tip of their left index finger was medically amputated.
An employee was tasked with checking a centrifuge on the centrifuge stand on a drilling rig. While descending the stairs, the employee slipped and fell to the floor, sustaining a broken bone in their foot, a broken ankle, and back and chest pain.
An employee was loading a roll of rubber mat into a pickup truck. The mat fell and caught the employee's finger against the bed rail. The employee suffered an amputation.

The employee was hospitalized due to chemical exposure.
