On April 25, 2018, an employee was driving a side loader when the brakes locked and the loader abruptly stopped. The employee fell to the ground and fractured both wrists.

On October 11, 2021, an employee was using a magnet to load a piece of 3/8-inch material onto the entry rollers of a machine. The load fell off the magnet and onto the employee's left foot, breaking multiple toes. The employee was hospitalized.

An employee was waiting for material to be loaded onto a processing table. After the light gauge material was loaded, the material expanded and struck the employee on their lower right leg. The employee's lower right leg was broken.

An employee was being trained to operate a yard truck to move trailers in and out of docks and to place them in storage areas. The employee was standing inside the yard truck cab as the trainer demonstrated how to operate the yard truck. As they performed a tug test, the truck jolted forward, causing the employee to sustain a lower back strain. The employee experienced numbness of the toes, bilateral leg weakness, and back pain that required hospitalization.

An employee was in a boom lift when the vehicle struck a hole then stopped abruptly. The impact ejected the employee and his fall protection pulled him back into the basket. The employee possibly struck the basket and was hospitalized for a leg fracture that required surgery.

Mack's Inc.

An employee was operating a three-wheeled forklift to hoist metal material with a sling system. While driving downhill in a gravel driveway, the front tire caught a root and the forklift lost momentum. This caused the hoisted material to sway and the forklift tipped forward. Inside the forklift, the employee struck against the dash and roof of the cage, sustaining a contusion to the forehead, abrasions to both legs, and later had a seizure.

An employee was moving gravel with a skid steer. The skid steer lunged forward and the employee fell backward. The employee's feet were caught between the standing plate and the framing above it, and both ankles were broken.

An employee was moving a bulldozer from one portion of the site to another. The bulldozer traveled over a mound of dirt. The blade got caught in the dirt on the opposite side of the mound and the dozer came to a sudden stop. The driver was propelled out of his seat and his head struck the top of the cab. The employee suffered two fractured vertebra and weakness in his right hand and leg.

A load of steel angles was being lowered when the lifting chain hit the load, causing it to slide toward an employee. An angle slid and pinched the employee, catching both of their legs between the beams and an angle. The employee suffered two fractured legs.

Ryder System Inc.

An employee was on the deck of a trailer helping a customer unload tubes. The employee was working to place a cargo strap on the remaining tubes using a winch bar when it slipped off the tubing, causing him to lose his balance and fall headfirst onto the asphalt lot below. The employee suffered a contusion on his forehead, resulting in hospitalization.

EASTERN METAL SUPPLY, INC.

An employee and a customer were unloading two rectangular tubes. The tubes were wrapped together. The plastic wrap was cut so that one tube could be picked up. As the employee was lifting the top tube, the other tube (still connected to the top tube) came down onto his hand and broke two of his fingers. He was hospitalized.

TEXAS PIPE AND SUPPLY COMPANY

An employee was assisting as a forklift got pipe from a pipe rack. The employee was on one of the forklift's forks as it was raised to the top of the rack, a height of about 15 feet. He fell to the ground and suffered a dislocated and fractured right elbow.

On September 23, 2025, at approximately 7:43 p.m., an employee was using a knife to cut poly banding from a metal coil. The knife slipped, causing a laceration to the inside of their left arm at the elbow joint. The employee was hospitalized and required surgery to repair a damaged vein.

An employee was driving a boom lift (in the lowered position) in an exterior dock area. The lift s left wheels rolled onto base plates that covered a 3-foot-deep sump pit. The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.
