An employee was driving a side-boom operator across a temporary bridge. The bridge collapsed and the machine rolled slightly and came to rest at about a 45 degree angle. As the employee disconnected his seat belt, he fell from the machine and landed in the creek bed on his back. The fall caused hairline fractures to his scapula and a rib.

While disconnecting a strap at the back of a flatbed truck trailer, an employee fell from the side of the truck, landing on wood mats about 5 feet below. The employee suffered a broken nose, broken ribs, broken teeth, fractured sinus cavity, fractured wrist, and facial lacerations. The employee was hospitalized.

An employee was steadying a road boring machine as it was loaded onto the back of a truck. The employee's right index finger was in a pinch point when the machine was lowered onto the finger, crushing it and requiring a medical amputation.

An employee was attempting to separate a hex socket from the sleeve of an adapter that was used to link a bore machine to an auger. The employee was supporting the hex socket with a finger while tapping the socket with a small hammer to unseat it from the sleeve. The socket gave way, amputating the employee's fingertip with a guillotine action.

On 11/4/2017, an employee was dismounting a bulldozer when the employee's right foot slipped. The employee then made hard contact with the ground, breaking the tibia and fibula above the right ankle.

An employee was running pipe into a hole on a rig floor. He was still running the other joint into the hole when the laydown machine shot the joint of pipe. The pipe hit the employee in the jaw and he fell backward, suffering a fractured cheekbone.

As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.
