An employee was reversing an electric pallet jack and backed into the corner of a storage rack. She suffered a fractured ankle, requiring hospitalization.
An employee was walking near a machine that was in operation when he noticed a loose wire on a large reel as the reel was rotating. The employee was caught by the loose wire causing the reel to flip the employee over and onto the ground resulting in a concussion and whiplash to his body and neck.
An employee had climbed onto warehouse racking and stepped off the racking onto a pallet that was being carried by a forklift. The pallet broke and the employee fell 9 feet with the pallet falling on top of him resulting in fractured vertebrae and 10 stitches.
An employee was operating the shrink wrap machine. As he was lowering the stretch wrap to the bottom of a pallet, his left ring finger was caught by the spinning plate that the pallet was sitting on. His finger then became caught in the machine and was amputated.
An employee was training on an rolling container carrier on the dry, non-frozen storage side of the distribution center. She made a turn around the end cap and her right foot struck the rail protecting the endcap racking. The employee's foot was fractured.
An employee was a passenger in a utility vehicle that was driving up a hill on a wooded trail. As the vehicle made a right turn, three fingers on the employee's right hand were caught against a tree and fractured.
An employee was adjusting a cable reel that was loaded onto the forks of a forklift. The reel shifted and caught the employee's left index fingertip against the backrest, amputating it.
An employee was on an extension ladder, about 15-18 feet high, while spraying a wasp nest on a telecommunications pole. He fell to the ground and sustained fractures to his hip, requiring hospitalization.
An employee was working from a ladder, pulling a cable line through a wall at a customer's house. He lost his balance and jumped off the ladder, landed on the ground about 7 feet below, and suffered injuries to both ankles including a fracture to at least one. The employee was hospitalized.
An employee was pulling inner duct from one handhole to another. The shackle on a winch cable contacted the cable sheave's roller block. The employee released tension on the winch cable and lifted the cable to guide the shackle over the lip of the handhole. As he re-engaged the winch, his left index fingertip became caught between components in the winch's retrieval wheel. The fingertip was amputated between the last knuckle and the nail.
An employee was changing a die in a press when the die slipped and crushed the employee's left index finger. The employee sustained an open facture of the tuft of the left distal phalanx and a partial amputation.
An employee was driving a boom lift (in the lowered position) in an exterior dock area. The lift s left wheels rolled onto base plates that covered a 3-foot-deep sump pit. The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.
