On April 27, 2025, employees were performing a railcar switch. When the train began to shift back, the injured employee's right arm got crushed between the knuckles of two railcars. The employee was hospitalized with fractures to their forearm.

On January 8, 2023, at approximately 2:20 AM, an employee was accessing a drag conveyor when his right hand contacted the running conveyor. He sustained amputation of his right index and middle fingers.

On August 7, 2022, at about 12:30 PM, an employee was working to open a gate at the bottom of a railcar when they were struck in the face with a metal prybar. This resulted in a laceration below the right eye, and an injury to the right eye that required surgery and hospitalization.

On August 30, 2021, an employee was opening the door to a pallet magazine while operating a forklift. As the employee backed up the forklift, the employee's fingers were pinched between its frame and the magazine door. The employee suffered a fingertip amputation.

An employee was on the ground by a railcar attempting to unplug an oil line to allow the oil to flow from the railcar to a tank in the facility. Oil splashed out of the top of the railcar and landed on the employee causing burns to the top of the employee's shoulders.

An employee was preparing a tank car for loading while chipping away solidified product in the manway opening of the tank car. A pressure noise within the tank car startled the employee, causing him to stumble backward and fall approximately 10 feet over the guard railing at the chemical loading rack to the ground. The employee suffered fractured vertebrae, a fractured right wrist and ankle, and a dislocated jaw.

An employee was installing an electrical tray remover from a ladder. As he was descending, his missed a step and fell 6-7 feet to the floor. The employee suffered a head injury.

Two employees were installing spray foam insulation in the attic of a new residential construction. The injured employee fell 8 feet from an A-frame ladder to the floor. The employee suffered lacerations, abrasions, and a head injury.

An employee was on a raised platform attaching wires to electrical boxes. The employee fell approximately 8 feet to the floor below and struck an exit sign on the way down, resulting in an injury to his face.

An employee was in a forklift man basket fixing a garage door opener when they fell approximately 8 feet to the concrete ground below. The employee suffered head and back injuries as well as kidney bruising.

An employee was working on removing the gearbox from the drive shaft of the motor assembly of a feather drier. He was using a hydraulic tool to pull the pieces apart when a bolt dislodged from the eye of the gearbox and the corner of the gearbox broke off. The employee was struck in the nose (sinus cavity), resulting in hospitalization and surgery.

An employee was on a forklift checking the sprinkler system drain. The employee fell 8 feet from the forklift to the floor. The employee suffered a broken collar bone and was hospitalized.

An employee was working as a dumper on a packaging line. A passing forklift turned and pinned the employee against a guardrail. The employee suffered fractures to their ribs and a leg requiring hospitalization.

An employee was cleaning an upper mezzanine. When he leaned against the mezzanine gate, he fell through the gate opening and landed 11 feet below. The employee sustained fractured orbital bones.

An employee was assigned to clean a feed mill in the basement. He then rode a tool and material basket to return from the basement to the ground level. He fell out of the basket and landed about 5 feet below. The employee sustained fractures to his arm, lower back, and foot.

An employee fell while moving a water canister, resulting in fractures to four fingers on her right hand.

An employee was changing a die in a press when the die slipped and crushed the employee's left index finger. The employee sustained an open facture of the tuft of the left distal phalanx and a partial amputation.

An employee was mixing sauce using a sauce barrel mixer when his left little finger was amputated.

An employee was driving a boom lift (in the lowered position) in an exterior dock area. The lift s left wheels rolled onto base plates that covered a 3-foot-deep sump pit. The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.

A temporary employee was carrying a nail gun through the warehouse when it hit his leg, activated, and fired a nail into the leg.
