An employee was assembling a large gearbox using the lifting hoist. Each end has swivel hooks hooked onto the assembly on the shaft ends. When he went to hook the hoist to the customer end the hook swiveled with his gloved hand on the chain hoist. As the weight shifted, his left index finger was caught between the chain and the chain adjustment resulting in amputation up to the first distal joint.

An employee was assisting with pipe on a drilling rig. The slips came down and landed on the employee's left foot fracturing four metatarsals.

Two employees were struck by the rotary table of a drilling rig while installing caisson. Employee 1 suffered multiple fractures, internal injuries, and head trauma. Employee 2 suffered multiple fractures, internal injuries, spinal injury, and head trauma.

Employees were felling hazard trees. The injured employee was conducting 'bucking' operations to saw a tree from the road when the tree kicked-back, pinning him against the ground and fracturing his left femur. The employee was hospitalized and required surgery.

An employee was greasing tongs on a work-over rig. The tongs lacerated one of the employee's fingers.

An employee was chaining a vehicle when he had to change tools and the compressor. As the compressor was being charged, the employee stepped near the vehicle to make sure the compressor was attached. Then, he instructed a co-worker to lower the compressor. His right hand was on the compressor and under the chain, causing his ring finger to be crushed.

An employee was moving a rack onto a lower conveyor in the south heat treat area when their finger got caught between the part tray and the rollers, resulting in a fingertip fracture and amputation.

An employee was operating an overhead crane using a remote control. They went to control the movement of a suspended load and their hand became caught between the suspended load and other objects. The employee sustained amputation of their left little finger.

An employee was working on an extruder line and changing the spool when the line became tangled. The employee went to remove the line from the roller when his left hand was pulled into it, becoming caught between the line and the roller. The employee sustained amputations of his index, middle, and ring fingertips.

An employee was preparing for a quality audit when they slipped on oil on the floor and fell to the floor. The employee was hospitalized with a leg injury.

A crane was lowering a piston into a carriage slot. An employee was guiding the piston when his glove got caught under the piston and his left middle fingertip was amputated.

An employee was moving a tensile strength test device with a dolly. The device fell from the dolly onto the employee's right leg, causing a compound fracture.

An employee was walking past a crossover conveyor when a ramp came down and hit them in the back. The employee sustained a broken back vertebra, as well as a concussion, and was hospitalized.

An employee was using a hook tool to pull a pallet onto the lift gate of a tractor trailer. The hook detached from the underside of the pallet, causing the employee to fall backward off the lift gate. The employee landed on the concrete parking lot about 5 feet below, suffering fractures to the skull and two thoracic vertebrae.

A skid loader backed over an employee's leg, breaking it.

An employee was walking on a truss table when he lost his balance and fell approximately 3 feet to the floor. The employee sustained a dislocated and fractured left ankle.
