On 05/23/18 at 3:00 p.m., an employee was assisting in pile driving operations. A drill bit was removed from the pile, and the employee's left thumb was caught in a pinch point on the hammer drill, resulting in an amputation.
An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool.
