An employee was working on a utility pole from an aerial lift bucket. He fell from the bucket to the concrete sidewalk below, suffering a laceration to his head, a fractured shoulder, and broken ribs. Fall protection equipment was not worn at the time of the incident. In addition, the safety chains on the door to the aerial lift bucket were not engaged.

An employee was starting to climb a utility pole after attaching the lower removable rung assembly. When the employee stepped on one of the rungs, it gave way and locked in place. The employee fell backward to the ground 4 feet below. The employee sustained back pain between the shoulders.
An employee was on an A-frame ladder retrieving materials from a shelf. He slipped and fell and his leg got caught in the ladder rungs, breaking his left tibia.
