On June 14, 2018, at 11:40 a.m., an employee was installing rebar bands on a gabion wall when he suffered a seizure, requiring hospitalization. Dehydration contributed to the seizure. The employee was tied off at the time so there were no physical injuries.

On May 17, 2022, an employee suffered a heart attack while performing tire repair at a customer's location.

An employee had been stripping foundation form work. The employee was in the break area when he got up to get a drink of water and collapsed due to a heart attack.

An employee was bent over while standing on the ground installing fence components. The employee collapsed and was hospitalized for a heart attack.

HB Fleming, Inc.

A crew was installing drilled micropiles alongside an outdoor covered deck foundation. The injured employee was working the front of the drill when a loose section of casing dropped onto the tip of a rig wrench and pulled the wrench down to an embedded casing. The employee's left little finger was caught between the rig wrench and the embedded casing and was amputated above the top knuckle.

Takkion OPS Management

An employee was standing on an extension ladder, using a torquing tool to remove bolts that secured blades to a rotor. When the torquing tool activated, its reaction arm came around and pinched the employee's right middle finger against a lifting eye. His fingertip was amputated.

Ahtna Marine and Construction Company, LLC

Employees were securing the fuel line of an outboard motor in preparation for removing the motor from a small vessel located in the equipment yard. A gasoline-related fire occurred and one employee sustained burns to both hands and forearms.

Michels Construction, Inc

A marine diver/construction worker was working underwater, using a hydraulic dredge to clear sediment away from the base of a dam. The dredge's suction nozzle began migrating toward him, and then pulled in his right hand and forearm. He sustained compartment syndrome in the hand and forearm.

Bechtel Energy Construction Services, Inc.

An employee was standing on a sheet of -inch by 4-foot by 8-foot plywood on top of a rebar mat, supervising his crew. As he was moving to alert the crew to an incoming overhead load, he stepped off the plywood and his left foot slipped through the rebar mat and landed on the rebar mat below. His left ankle was broken and dislocated.

An employee delivered a package. On her way back to her vehicle she was attacked by two dogs that came from around a corner. She was hospitalized with dog bites.

AHTNA Marine & Construction

The injured employee was standing by the controls for the electric spud system on the barge, preparing to raise the spuds. Meanwhile, an excavator was moving a mat on the deck. As the excavator swung from port to starboard, it's counterweight struck the injured employee s shoulder, causing them to lose balance and fall against the spud controls. The employee sustained bruises and contusions to their right shoulder and chest.

Fairbanks Morse, LLC

A technician was operating a crane hoist to lift/remove a cylinder head from the main engine (port). The load was lifted at an angle, causing the cylinder head to get stuck on the cylinder head studs. When it became free from the studs, the cylinder head swung. It struck the technician and caught them against the cabinet door. This resulted in an amputation of their left index fingertip at the distal knuckle.

Container Maintenance Corporation

An employee was securing a power cord to a refrigerated truck container. A truck started backing up to connect with the container and the employee was pinned between the tires of the container and the bolster of the truck, resulting in a fractured leg.

An employee was delivering a package to a house when a dog came through the storm door, jumped on the employee, and bit them in the lower jaw.
