An employee was operating a pasta manufacturing extrusion press when his arm entered one of the bins where pasta flour was being mixed to push the pasta remnants into an extrusion bore. The press paddle lacerated his right bicep area. The press' safety gate was in place at the time of the incident.

An employee was standing on an extension ladder, using a torquing tool to remove bolts that secured blades to a rotor. When the torquing tool activated, its reaction arm came around and pinched the employee's right middle finger against a lifting eye. His fingertip was amputated.
