An employee was helping another employee put a block under an aluminum mold machine. After he stepped away from the machine, another employee began to turn it on; noticing that the block had shifted, the first employee tried to adjust it. The machine smashed his right index and middle fingers (causing severe lacerations), as well as the tip of his ring finger.

An employee was using a ladle to pour molten aluminum into a tilt when he started feeling ill and then vomited. He later experienced aching arms and hands, and was hospitalized for severe dehydration.

On December 19, 2023, an employee was setting up a copper wire drawing machine when his left ring finger was pinched between a copper wire and a machine part. The employee's finger was partially amputated.

An employee was evaluating a coiling machine to verify that the blade was present inside it. The machine's weld function was activated, and the machine crushed the employee's fingers, resulting in the amputation of the left middle and ring fingers.

On July 29, 2025, an employee working near a metal furnace. Toward the end of his shift, he was charging a furnace and began to experienced heat exhaustion. He had also been in areas of elevated temperatures due to heat sources such as direct sunlight and a combustion engine. The employee was hospitalized with dehydration.

An employee was using a pry bar to remove a piece of aluminum cone from a mold on the cone line. The cone broke loose and the employee's left middle finger was caught between the pry bar and the cone line frame. The employee's fingertip was partially amputated.

An employee was machining a trunnion using a grinding rig when their left hand was caught between the grinding stone and the trunnion, resulting in fractures to the middle, ring, and little fingers, as well as severe damage to the skin and amputations of the middle and ring fingers to the distal knuckle.

On August 7, 2024, a maintenance technician was descending a fixed cage ladder when the ladder broke and they fell approximately 12 feet to the concrete ground. The employee sustained head injuries and fractured ribs.

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

The Goodyear Tire & Rubber Company

On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.

Big River Steel, LLC

An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.

American Tile & Marble Co., Inc

An employee was helping two other employees to push a pallet of tile on a loading dock. He suffered a broken fibula and was hospitalized.

On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.
