An AECOM maintenance employee was troubleshooting a malfunctioning encoder device. To access the device, the employee had to stand on top of a tank holding a caustic cleaning solution, about 3 feet off the ground. The employee's left leg slipped through one of the access covers and was immersed in the caustic solution up to the knee. The employee suffered third-degree burns to the lower leg.

An employee was welding on a newly fabricated fuel tank when a flash fire occurred. Residual fumes in the tank ignited and flames exited the tank opening, causing burns to the employee's face and chest.

An aircraft mechanic was conducting maintenance on an aircraft from the ground. When he extended a boarding ladder, which is built into the aircraft, his left thumb was caught in pinch points and amputated from the nail to the knuckle.

While surveying in the middle of an intersection, an employee was struck by a vehicle and suffered broken ribs.

An aircraft mechanic was working on the left wing of an F-18 aircraft removing a component section of the wing known as a flight control surface. During this process the employee pushed on a bolt with his finger. When the bolt moved, the employee's right index finger followed and was caught in a pinch point by the weight of the falling component. The fingertip was amputated.

An employee was on a step ladder applying glue to a panel when the ladder fell apart, causing the employee to fall to the floor. The employee sustained rib fractures.

On December 5, 2023, an employee was loading product onto a freight elevator when the elevator fell with the employee inside. The employee sustained a right leg injury.

On 11/28/2023, an employee was loading a cart with supplies to be delivered to a restaurant. He stepped onto a ramp to get into the back of a truck but the ramp fell to the ground, causing the employee to fall as well. The employee was hospitalized with a fractured ankle.

An employee was climbing a 16-foot fiberglass extension ladder to access a roof so they could take measurements for a roofing quote. The employee had partially ascended the ladder when the ladder broke in half. The employee's foot was caught in a ladder rung resulting in an ankle fracture that required surgery.

An employee was replacing a heating, ventilation, and air conditioning (HVAC) valve in a ceiling while standing on a ladder. The ladder collapsed while the employee was descending, causing them to fall to the floor. The employee sustained a broken wrist.

An employee was on a four-step work stand while sanding an area of an aircraft. He was climbing down the work stand when he tripped over an air hose and fell to the level below, resulting in a fractured femur.

An employee was lifting a manhole cover when it fell on his right foot, resulting in the amputation of his big toe and the adjacent toe.

A Bureau of Land Management (BLM) employee was assisting a fire crew with clearing a forest trail. The employee was struck in the left leg by a fire-rated chainsaw. The chainsaw struck the employee underneath their chaps and above their boot. The employee was hospitalized and required surgery.

An employee handed a fuel hose to another employee at a gas station pump and then turned around, tripped over the hose, and fell to the ground. The employee was hospitalized with a fractured hip.

An employee was conducting fire suppression operations on a wildfire. He pulled a hose from the engine. He then charged the hose and began to put on his gloves. A flareup occurred and the employee sustained burns to his face and hands.

An employee was on a 6-foot A-frame ladder, working to replace ceiling tiles. They fell to the ground and sustained fractures to their hip and femur. The employee was hospitalized and required surgery.

An employee was installing shoring and stepped on a 2x4 that was running across the top of a trench. He fell through and was hospitalized with a fractured left femur.

An employee was taking a break from powerwashing a deck when a door closed on his left index fingertip, resulting in an amputation above the nail.

An employee was securing a ramp to a dock to unload cargo. He was using large pins to put the ramp in place when two of his fingers got caught in the ramp, resulting in a partial amputation.

An employee was helping to dismantle a mobile crawler crane. Employees were separating boom sections. When the last pin was hammered out, the 40-foot boom section dropped 4-5 inches onto the injured employee's left foot. All five toes were amputated and the employee was hospitalized for surgery.
