Federal OSHA recorded a severe workplace injury at AECOM, Naval Air Station Oceana, VIRGINIA BEACH, VIRGINIA 23460 on — Amputations, affecting the fingertip(s).

An aircraft mechanic was working on the left wing of an F-18 aircraft removing a component section of the wing known as a flight control surface. During this process the employee pushed on a bolt with his finger. When the bolt moved, the employee's right index finger followed and was caught in a pinch point by the weight of the falling component. The fingertip was amputated.

AECOM

An AECOM maintenance employee was troubleshooting a malfunctioning encoder device. To access the device, the employee had to stand on top of a tank holding a caustic cleaning solution, about 3 feet off the ground. The employee's left leg slipped through one of the access covers and was immersed in the caustic solution up to the knee. The employee suffered third-degree burns to the lower leg.

An employee was welding on a newly fabricated fuel tank when a flash fire occurred. Residual fumes in the tank ignited and flames exited the tank opening, causing burns to the employee's face and chest.

An aircraft mechanic was conducting maintenance on an aircraft from the ground. When he extended a boarding ladder, which is built into the aircraft, his left thumb was caught in pinch points and amputated from the nail to the knuckle.

TForce Freight

An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool. The dock plate slipped and the employee went to adjust the plate with their hand when their left middle finger got caught. Their finger was amputated above the first joint.

Liveo Research, Inc.

An employee was rotating a carbide granulator blade when his finger was caught between the blade and a stationary part of the machine. The employee's right middle finger was partially amputated and they suffered an avulsion to the right ring fingertip.

An employee was using a CNC metal lathe machine in manual mode to check that the thread blocking tool made contact with the jaws. As the employee was manually spinning the spindle, his index fingertip was crushed between the spindle's jaws and the grooving insert, resulting in a fingertip amputation.

During a material handling operation, an employee's fingertip was caught under an air conditioning unit as the unit was lowered onto a cart. The employee's fingertip was amputated.

An employee was removing a bolt from a tractor hub when their right index finger was caught between the hub and the frame. The employee's fingertip was crushed and the employee was hospitalized.

The Boeing Company

Two employees fell from an access bridge while entering an aircraft to perform maintenance. They landed on the ground about 20 feet below, where one of them lost consciousness. One employee sustained a fractured knee and another employee sustained a back strain/sprain. Both employees were hospitalized.

United Airlines

An employee experienced sharp pain in his lower torso area while loading baggage into the front cargo pit of an aircraft. The employee sustained a hernia.

M1 Support Services

The injured employee was assisting in the disassembly phase after a radar array had been lowered and secured. The crew began removing load bearing pins from an overhead crane to free the radome. A load bearing pin was stuck. The injured employee went to remove the pin manually as a second team member applied pressure from the opposite side. The pin unexpectedly released and struck the injured employee s right thumb, resulting in partial amputation of the distal phalanx including an open distal phalanx fracture and nail bed laceration.

FSM Group LLC

On September 11, 2025, an employee was lowering a railing system on an aircraft fueling cart when his gloved right ring finger was caught in the folding railing system, resulting in a fingertip amputation that required hospitalization and surgical reattachment.

Certified Engines Unlimited, Inc.

An employee was working at ground level when an aviation engine cylinder weighing 30 pounds fell from a seven-foot high shelf and struck the employee on the head. The employee suffered a fractured skull and was hospitalized.

AAA Complete Building Services, Inc.

A building engineer was inspecting a generator. As he closed the generator enclosure door, his right ring finger was caught between the door and the door frame. The fingertip was amputated.

U.S. Postal Service

An employee was walking back to a carrier case with mail in her hand when she tripped over a tub. Her back overarched as she fell, resulting in a fractured back that required hospitalization.

Virginia International Gateway Terminals, LLC

An employee was driving a utility tractor rig when it struck the rear trailer of another utility tractor rig, which was traveling in the opposite direction. The glancing impact caused damage to the cab, and he sustained a laceration on right side of his face, a cervical fracture, two fractured left ribs, and a pelvic fracture. The employee was hospitalized.

L3Harris Technologies, Inc.

An employee was washing an aircraft from a scissor lift when they fell to the floor. The employee sustained a back injury and lacerations to their head and neck.

An employee was working with a drill and a metal cable. The drill caught the cable and caused an amputation to the employee's left ring finger.
