On February 12, 2025, an employee was performing maintenance on an extruded profile roller. The employee was caught in the powered roller assembly and suffered finger amputations on his left hand.
An employee had been stripping block outs from the form of a precast concrete double tee. Their left hand became caught between the concrete and the steel form, resulting in a severe laceration and crush injuries to their left index, middle, and ring fingers. This resulted in an amputation of the left index finger from the tip to the distal joint, the left ring finger from the tip to the fingernail, and the medical amputation of the middle finger to the distal joint.
An employee was riveting two pipes together using a rivet machine when the rivet hammer activated and crushed the employee's left index finger. The injury resulted in an amputation.
