On February 12, 2025, an employee was performing maintenance on an extruded profile roller. The employee was caught in the powered roller assembly and suffered finger amputations on his left hand.

An employee had been stripping block outs from the form of a precast concrete double tee. Their left hand became caught between the concrete and the steel form, resulting in a severe laceration and crush injuries to their left index, middle, and ring fingers. This resulted in an amputation of the left index finger from the tip to the distal joint, the left ring finger from the tip to the fingernail, and the medical amputation of the middle finger to the distal joint.

When the employee rolled the part from vertical to horizontal, the pipe side of the manifold slipped off his hand. An employee was moving a piece of an I-beam for welding when it rolled off the cribbing. The employee went to catch it when it fell and crushed the employee's finger, resulting in a partial amputation.

An employee was helping to disassemble large reels used to store steel wire when a 264-pound flange from the reel fell over onto the employee's left leg. The employee sustained multiple fractures to their leg and ligament injuries to the lower leg.

The injured employee was putting away materials on the warehouse floor as a co-worker was pushing a pallet onto a nearby shelf. The pallet knocked another pallet forward, causing it to fall onto the injured employee's shoulders. The employee suffered fractures in their right and left knees and ankles.

Employees were moving a single man lift into a building and reclining the lift to position it to fit through the door. The lift shifted and fell, causing the employee to sustain fractures to the left tibia and fibula. The employee was hospitalized and had surgery.

An employee was moving a light tower so it could be hitched to a truck. He grabbed the tongue of the trailer hitch on the light tower to slide it to the left. The tongue jack fell off, causing the light tower tongue to drop on the employee's right hand. The employee's middle fingertip was amputated.

A steel tube fell from a straddle truck onto an employee's foot, causing the steel toe of the employee's shoe to press into the employee's left foot. The employee sustained fractured phalanges and was hospitalized.

An employee was unloading materials from the back of a flatbed trailer. The employee fell off the back of the trailer approximately 4 feet to the ground below, resulting in a fractured left collarbone and fractured ribs. The employee was hospitalized.

An employee was bending material in a mechanical brake press when the press rotated and caught his right hand. His finger was crushed between the die and a steel plate, resulting in amputation of the finger.

Precision Custom Components, LLC

An employee was cleaning pans containing adhesive by pouring acetone into the pans. After soaking, the employee used a plastic spatula to scrape the paint from the pan. During scraping, a fire spontaneously started underneath the employee near the pan. The employee threw the container of acetone out of the way to avoid it catching on fire, and spilled acetone onto his clothing, causing the fire to spread. The employee suffered burns to both hands and the right calf.

The injured employee was assisting a co-worker to transfer a V-baffle (large piece of angle iron weighing 736 lbs.) into a vessel shell. The injured employee stepped off the end of the vessel and fell backward. The employee sustained a dislocated left shoulder, a fractured left wrist, a fractured lower right leg, and a laceration to the left eyebrow.

RG Professional Carpenter, Inc.

The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.

An employee was on a ladder, taking measurements for a new gutter installation. The ladder slipped out from under him and both he and the ladder fell to the ground. He was hospitalized with seven broken ribs and a broken clavicle.
