An employee was operating a compactor truck in a parking lot. The compactor tipped over; the employee was thrown out, then its rollover protection system struck his right leg, causing an amputation injury. A seat belt was not being worn at the time of the incident.
An employee was operating a sit-down type of material handling equipment (MHE). They were turning the MHE with the clamp attachment in the air when the MHE fell over onto its right side.
