On August 13, 2020, an employee was removing a filter screen from a washer machine. While the employee's right ring finger was between the screen and the filter housing, the edge of the filter screen struck it and amputated the fingertip.

An employee was loading empty water bottles onto a truck using a dolly. A bus hit the dolly, which then struck the employee and fractured his collarbone.

An employee was going to the breakroom when they tripped over a chair and fell, resulting in a hip fracture.

An employee arrived at work and was walking into the store through the parking lot. The employee tripped over a curb and fell to the ground, sustaining a femur fracture.

A mechanic was performing a modular overhaul on a blower. While removing magnetic rods for the stretch rod, the employee was placing the second rod on the blower platform. The rods were magnetically attracted to one another, and the employee's left third and fourth fingers were pinched between them. She was hospitalized and sustained a partial amputation.

An employee was conducting maintenance on the bag in box (BIB) palletizer machine when the motor chain jumped, pulling his right index finger into the chain drive attached to the motor and causing a fingertip amputation.

An employee was clearing a piece of tape from the tape dispenser when their hand contacted the cutting implement on the dispenser. The employee suffered the amputation of their right index finger.

The employee went to check for pre-blow air and the nozzle dropped back down to its starting position, catching his right index fingertip against the mold base. The employee sustained a fingertip amputation to the first digit.

An employee was attempting to clear a bottle jam when they were pinned to the wall by the arm of a robotic bottle stacker. The employee sustained an abdominal injury that required hospitalization. The machine was not guarded at the time.

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.

An employee was installing fence posts when they stepped into a recently dug hole that was not visible due to rainfall. The employee's leg was fractured.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee was inspecting a breaker box and turning on/off a circuit breaker when he was electrocuted.

An employee was walking on the sidewalk. When they stepped off the curb, they fell to the ground, resulting in fractures to their tibia, fibula, and a metatarsal.
