Final narrative
An employee was installing a gas shock on the lid of a large box in the dress-out area. The box requires two shocks and after installation of the first shock, another employee asked the injured employee to move the box that he was working on. The injured employee was in the process of moving the box when the lid closed and caught his hand between the lid and the box frame. The employee suffered an open left thumb distal phalanx fracture with a nailbed injury.
Two employees were pulling the straps from cylinders prior to putting them in the oven. Employee 1 stepped off a platform and hit a shelf, which dislodged the mold that struck both employees in the feet. One employee sustained a fractured left foot, and the other sustained six fractured toes and a partially amputated right big toe. Both employees were hospitalized.
