Federal OSHA recorded a severe workplace injury at ALTEC INDUSTRIES, INC., 2106 SOUTH RIVERSIDE ROAD, SAINT JOSEPH, MISSOURI 64507 on — Fractures, affecting the ankle(s).

An employee was on the back of a digger derrick, prepping the unit for an endurance test. The employee was using a torque wrench to remove bolts for analysis. As the employee was applying force, the socket slipped from the bolt resulting in a loss of balance. The employee fell 4' 7" from the back of the truck onto the ground. The employee landed on both feet and sustained a broken left ankle that required surgery.

ALTEC INDUSTRIES INC

Two employees were pulling the straps from cylinders prior to putting them in the oven. Employee 1 stepped off a platform and hit a shelf, which dislodged the mold that struck both employees in the feet. One employee sustained a fractured left foot, and the other sustained six fractured toes and a partially amputated right big toe. Both employees were hospitalized.

An employee was moving upper booms when their right hand was caught in a pinch point between the hoist hook and the lifting attachment, resulting in a partial amputation of their thumb.

The injured employee was assisting another associate in working on a bumper winch. The injured employee's hand was on the bumper when someone under the truck called the employee's name. The injured employee looked under the truck and the eyelet on the bumper winch caught their thumb. The employee sustained a partial amputation to the right thumb.

ALTEC INDUSTRIES, INC.

An employee was installing a gas shock on the lid of a large box in the dress-out area. The box requires two shocks and after installation of the first shock, another employee asked the injured employee to move the box that he was working on. The injured employee was in the process of moving the box when the lid closed and caught his hand between the lid and the box frame. The employee suffered an open left thumb distal phalanx fracture with a nailbed injury.

ALTEC INDUSTRIES, INC.

An employee was positioning a hoist with rigging equipment to move a boom. As the employee was raising the hoist cable and hook, the hook was caught on the side lip of a work table. The hook then broke loose and swung, striking the employee in the face. The employee suffered a facial laceration, broken jaw, and tooth extraction.

Electrical Contractors Inc.

An employee was climbing a step ladder while carrying a 3-foot piece of conduit. As he went to reposition his feet on the ladder by pivoting, he slipped and fell from the third rung of a 6-foot ladder. The employee sustained fractures to the left femur, right elbow, and right ring finger.

M1 Support Services

An employee was descending a 4-step maintenance stand when she missed the bottom step and fell to the hangar floor. The employee suffered a left hip fracture.

FEDEX Ground Package System, Inc.

An employee stopped a belt and was going to check it for missing packages. She fell backward 3-4 feet from an elevated platform and sustained a broken right arm.

SRM Concrete

An employee had just finished a routine concrete pour and was ascending the ladder to clean the concrete mixer truck. The employee lost their grip and fell approximately 2-3 feet, contacting the truck's bumper. The employee suffered rib fractures and a punctured lung.

U.S. Postal Service - Los Angeles P&DC

An employee was performing duties as an expeditor. After opening the dock door, the employee scanned the barcode on the door of the truck and placed one foot on the truck and one foot on the dock. The truck drove out of the stall, causing the employee to fall 4 feet off the dock onto the concrete. The employee sustained fractures to the right side of the pelvis, elbow, and a left ring fingertip as well as injuries to the right wrist and bruising to the back and stomach.

Riverside Industries LLC

An employee was working behind a stack of steel I-beams (stacked 2 columns deep and 4 levels high on the floor) while a forklift was pushing the stack. When the forklift took off the second-to-last beam, the last one was pushed over, causing it to fall to the ground and strike the employee. The employee sustained a fracture to the right leg.

Bobcat Company

An employee was lifting a flat sub (sheet steel) in the flats robot cell (weld area). He was operating a hoist when his left little fingertip got caught between the lifting device hook and the flat sub. The employee sustained a partial transphalangeal amputation.

Altec Service Center

An employee was underneath a 35-foot boom truck and adjusting the transmission linkage when the truck jumped over the wheel chocks and rolled over the employee's right leg, resulting in a fracture.

GRADALL INDUSTRIES, INC.

A crane was being used to lift a debris tank from a rack. The tank crushed the employee's right hand, causing amputations to the employee's right ring finger and little fingertip.

Manitou Equipment America, LLC

An employee was removing a small sheet metal part out of the die of the brake press. The foot pedal was pressed, causing the part to lift and crush their left thumb against the die. The employee suffered an amputation to the thumb tip including the loss of flesh and the fingernail.

WADDELL CONCRETE INC.

An employee was readjusting a safety hook to remove hardware from gang forms when his feet slipped. He fell to the ground, resulting in a broken left arm and wrist.

Dollar Tree

An employee was on a step ladder in a store aisle, making room on shelving for merchandise. She fell from the step ladder to the floor, suffering two fractures in her left leg. She was hospitalized, requiring surgery.

Corrigan Brothers Inc

An employee was closing the side cargo door to a company van. The tip of his finger got caught between the door and the door jamb, resulting in a partial amputation.

Plumb Supply Company

An employee lost his footing while climbing an inclined ladder. He stumbled down one step and was able to stabilize himself by holding the railing and twisting his body, but he suffered an internal abdominal injury and was hospitalized.

An employee was manipulating dies on a brake press. The press caught and crushed the employee's right forearm, resulting in hospitalization.
