An employee was moving upper booms when their right hand was caught in a pinch point between the hoist hook and the lifting attachment, resulting in a partial amputation of their thumb.
Two employees were pulling the straps from cylinders prior to putting them in the oven. Employee 1 stepped off a platform and hit a shelf, which dislodged the mold that struck both employees in the feet. One employee sustained a fractured left foot, and the other sustained six fractured toes and a partially amputated right big toe. Both employees were hospitalized.
An employee was on the back of a digger derrick, prepping the unit for an endurance test. The employee was using a torque wrench to remove bolts for analysis.
