An employee was on a platform, manipulating the inner diameter of a vessel with a hydraulic press using quarter-inch square steel tubing. The tubing became free and knocked the employee off the platform and onto the floor. The employee suffered injuries including contusions, lacerations, and broken ribs. The employee was hospitalized.

An employee was gathering a 3/4" thick, 3' to 4' in diameter piece of plywood from a stack of approximately 9 similar pieces of plywood when the stack tipped over and struck the employee, pinning him against a forklift. He sustained a fractured jaw, fractured ribs, laceration to the lip(s), and broken teeth, requiring hospitalization.
An employee was on an A-frame ladder retrieving materials from a shelf. He slipped and fell and his leg got caught in the ladder rungs, breaking his left tibia.
