An employee was assisting a press operator. When the power press engaged, it caught the employee's fingers in the machine resulting in amputations.

An employee was using an overhead crane to move a rack upright. The 200-pound rack fell from the crane and crushed the employee's left foot.

Another employee came into the work area with a torque gun and began poking the injured employee with the tip of the torque gun and then activated the tool. The spinning tip of the torque gun caught the injured employee's left glove and amputated her middle finger.

An employee was working on a hot strip mill when a co-worker approached from behind and pushed in the back of the employee's left knee, causing it to buckle. The employee fell to the ground and sustained torn ligaments in the left knee.

The employee was testing the "trigger" sensor of the machine and adjusting the blades when the machine actuated and a blade amputated part of his left index fingertip.

An employee was moving a large safe weighing approximately 625 lbs. with a dolly that was specially-designed to move the safe. The safe fell from the dolly, struck the ground, fell forward onto the employee, striking their right hip/pelvis area before rolling off the employee onto the floor. The employee sustained a right hip/pelvic fracture.

An employee was checking the industrial waste stream as it discharged into a 4,000-gallon waste collection tank. The employee's right wrist was splashed with corrosive chemicals, resulting in hospitalization for a chemical burn.

An employee was checking a pole-type transformer tank for leaks. A hydraulic clamping device was lowered onto his right thumb and crushed it against the sharp edge of the bottom of the tank. He suffered a partial transphalangeal amputation to the thumb.

The injured employee was standing behind a table. She jumped out to scare another employee, but tripped and fell on the tile floor, injuring her right ankle/foot. She was hospitalized with a dislocated ankle that required surgery.

An employee was going into a trailer to mark product for shipment and the trailer pulled away from the dock. The employee fell out of the trailer, contacted the dock plate, and then fell to the ground. The employee was hospitalized with a punctured lung, fractured ribs, and contusions.

An employee was hooking up bundled tie-downs with a chain. While he was holding a hook, the other hook was unlocked. This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle. He suffered an amputation to the fingertip (without bone loss), as well as an open fracture.

An employee was operating a gas tugger to lift metal sheeting. His left hand was pulled into the pulley, which crushed his left thumb, resulting in avulsions and other tissue damage. He was hospitalized and required surgery.
